Protecting Patients from Fall Risks

Employ systematic screening of patients at risk of falling, with specific and multidisciplinary care

Context

Falls rate among the primary causes of hospitalization for patients over age 65, and they also remain by far the most frequent adverse event that occurs during hospitalization. Patients who have fallen or are at high risk of falling should be given specific care to limit their fall risk. They should also be sufficiently informed of and made attentive to these risks and the behaviors to adopt in order to avoid them.

Project

The project consists of installing the CHEOPS program for systematic screening of patients admitted to the hospital at Trois-Chêne in order to identify those at risk of falling and the implementation of multidisciplinary care adapted to this patient population.

An explanatory brochure is available for patients
