Each year, more than 4,600 patients undergo rehabilitation at HUG to regain autonomy and quality of life. Many, however, express the need for greater clarity and active involvement in their care. Today, the Concerto app already provides access to personalized information and communication with care teams. MoPRéa goes further: it offers a dedicated digital module for rehabilitation. By centralizing the patient journey, schedules, goals, and exercises, it empowers patients to play an active and reassuring role. This project embodies a modern vision of healthcare, where every patient is a partner in their recovery.
Project
MoPRéa is a digital module integrated into Concerto, designed as a true rehabilitation companion. It guides patients through five phases—Informing, Preparing, Transfer, Rehabilitation, Discharge. With checklists, interactive diaries, personalized exercises via Wibbi, and notifications, the module makes every stage clearer and more motivating. Families can also be involved with selective access to key information, enhancing support at home. For caregivers, MoPRéa improves coordination and efficiency, while highlighting digital innovation in healthcare.
