Patients as Partners in Their Treatment Schedules

Context

Preparing for patient discharge is a process that begins on the day the patient is admitted; it requires not only strong coordination between healthcare professionals, but also the implementation of protocols in which the hospital stay is carefully planned and documented.
“Healthcare navigators” offer patients assistance in getting the individualized healthcare they need. They coordinate appointment scheduling, transportation, escorting, directions to specific divisions, etc. The recent profession of IPM (Patient Scheduling Manager) has arisen in this context.
 

Project

The project’s objective is to build an ongoing program, based on healthcare navigators and networks, for individualized care within the framework of the general internal medicine division. This project will begin with a pilot phase to allow it to demonstrate its impact on the quality, safety, and efficacy of care in close collaboration with the patient and the patient’s family.
