Context

Each year, around 400 patients are followed at HUG for a transplant or while awaiting transplantation. Their journey is demanding: it requires deep lifestyle changes, complex treatments, and moments of uncertainty. In this context, information is a vital ally. Yet, paper brochures and oral explanations quickly reach their limits in emotionally charged situations. Today, patients are asking for modern resources, accessible anytime, that help them understand, learn, and gain autonomy. With cognitive science and digital tools, we can transform the experience, reinforcing trust, safety, and empowerment. The project embraces this opportunity, making therapeutic education a true driver of autonomy and serenity.

 

Project

The project aims to develop a digital therapeutic education module integrated into the Concerto institutional app. Accessible 24/7, it will combine practical guides, audio and video testimonials, interactive quizzes, FAQs, and virtual environments. This module will support patients and their families at every stage of the transplant journey: before, during, and after hospitalization. By fostering better understanding of treatments and good practices, it will reduce stress, improve therapeutic adherence, and strengthen the bond of trust between patients, families, and healthcare teams. Driven by a multidisciplinary team and supported by patient partners, this project embodies concrete and human-centered innovation in healthcare.
